Health and Housing Case Management Services RHD case manager provides supportive services needed to address the barriers of consumers to stabilize clinically and secure housing. Remote Monitoring (Pilot Project) RHD provides the master lease, utilities, and furnishing; the Remote Monitoring Pilot Home will allow eligible individuals to experience independent living through the use of Assistive Technology. Contact Us

Coming Home (A Justice-Involved Permanent Supportive Housing Program) RHD, in partnership with Alliance Health, has collaborated with the Durham County Housing Authority, the Durham Local Reentry Council, and the Durham Criminal Justice Resource Center to create the Coming Home Program. Contact Us

Community Transitional Recovery Program (CTRP) is to provides short-term assessment and recovery supports to individuals who have high and complex needs exiting institutions or other acute crisis settings to further prepare them for community-based living and to plan for community inclusion. Wake County Bridge Housing consists of two locations, each including three bedrooms. Each participant will have their own private bedroom, refrigerator, and microwave as well as a shared full kitchen and living room space. Contact Us
