Community Transitional Recovery Program (CTRP) Mecklenburg County
The Community Transitional Recovery Program (CTRP) is a short-term, community-based mental health program designed to support individuals with high and complex needs as they transition from institutional or acute crisis settings back into the community. Our goal is to provide the necessary tools, supports, and personalized planning to empower participants to live as independently as possible in the least restrictive environment of their choosing.
This innovative recovery program serves as a stepping stone, helping people stabilize, regain confidence, and build the essential skills they need for long-term success in community living.
Program Overview
Participants reside in one of eight two-bedroom duplexes located in Charlotte, NC, with a maximum stay of 90 to 120 days. Each transitional home is thoughtfully designed to balance independence and support, fostering dignity, safety, and personal growth.
Supportive, Person-Centered Care
Our interdisciplinary mental health team includes a Program Manager, Case Managers, Peer Support Specialists, and a part-time Nurse. Staff are on-site daily from 7 a.m. to 9 p.m., seven days a week. During overnight hours, participants benefit from a remote monitoring system (no cameras), offering discreet safety features and access to live assistance at the touch of a button. Individualized Support Plans
Each participant receives a personalized recovery plan tailored to their goals, strengths, and needs. Services may include:
- Community navigation and travel training (public and on-demand transportation)
- Employment and benefit coordination
- Medication support and wellness planning
- Life skills development
- Social connection and community inclusion activities
- Supports in locating more independent or permanent housing
Our approach is grounded in the principles of person-centered care, with a strong emphasis on the social determinants of health and a housing first philosophy.
Planning for What’s Next
From day one, we begin planning for each participant’s transition to their next home in the community. This includes housing assessments, resource navigation, and ongoing coordination with care teams, psychiatric and primary care providers.
Eligibility Requirements
To be eligible for the CTRP program, participants must:
- Be able to self-administer medications
- Be able to live safely and independently in a setting with a roommate, unsupervised at times
- Be motivated to pursue more independent living
- Be 18 years of age or older
- Have a primary mental health diagnosis
- Be Alliance Plan Members

We work closely with hospitals, crisis centers, housing agencies, and community partners to ensure continuity of care and a successful transition for every participant possible.
