Community Choices
Community Choices supports individuals with physical and mental health challenges in a residential setting within Chester County, PA. The program works with individuals with “Housing First” and “Medically Fragile” needs with the goal of developing independence and working to move the individuals to a decreased level of care within the community through skill building.
