GENERAL
An employee is eligible for benefits if they work at least 80% of the unit's
full time hours. Full time employees working less than 100% of the unit
workweek will be prorated for benefit dollars. The effective date for benefits
is the first of the month following three months of full-time employment.
The benefit structure has two components. First, RHD provides benefit dollars
based on a percentage of salary. The employee may use these benefit dollars any
way they choose. In addition, to assist with health insurance premiums, RHD
pays a portion of those monthly premiums, from 30% - 100%,based on the level of
coverage. If they waive all benefits, the entire amount is added to their
paychecks, and taxed accordingly. If the benefits chosen cost less than the
allotted amount, the difference is added to their paycheck. If the benefits
chosen are greater than the benefit dollars, a salary reduction for the
difference will be taken from the employee's paycheck.
Here, in brief, is a list of insurance options available through the group
plan:
HEALTH INSURANCE
Comprehensive coverage, including prescriptions, is available in
all states.
DENTAL INSURANCE
Two levels of coverage are available.
TERM LIFE INSURANCE
Guaranteed issue to new hires, term life insurance is available
from $10,000 to $50,000. Rates are based on age.
ACCIDENTAL DEATH & DISMEMBERMENT
Pays a benefit to beneficiary in the event of accidental death
only, or pays benefit to employee for loss of limb. Less expensive than term
life insurance.
SHORT TERM DISABILITY
Pays a benefit to beneficiary in the event of accidental death
only, or pays benefit to employee for loss of limb. Less expensive than term
life insurance.
LONG TERM DISABILITY
Provides coverage when STD benefits stop. Premium is based on
employee's salary. Guaranteed issue to new hires. Pre-existing condition clause
for health treatments in the three months prior to enrollment.
FLEXIBLE SPENDING ACCOUNTS
FSA's are not insurance coverage. They are tax-free accounts
that can be used for specific medical, dependent care, and/or transportation
expenses not covered by insurance.
COVERAGE THROUGH COLONIAL LIFE
INSURANCE CO.
Supplemental Disability - In addition to the 60% of Short-term Disability,
Colonial Life offers the opportunity to purchase an additional 40% coverage,
thereby providing 100% of your salary should you be disabled.
Universal Life Insurance - If additional life insurance is needed, Colonial
offers supplemental policies based on your insurance needs.
Accident Insurance - Helps offset the direct and indirect expenses resulting
from an accident, such as co-payments, deductibles, and other costs not covered
by traditional health plans.
Cancer Coverage - Helps pay the direct or indirect costs of cancer. These plans
offer screening benefits that encourage regular health exams
DOMESTIC PARTNER
Most insurances cover domestic partners (same sex included) as
spouses. There is a domestic partner form to be completed, and three forms of
proof that the employee and partner reside together and are committed six
months or more. More information is enclosed.
RETIREMENT PLAN
RHD offers a 403b plan through Great West Retirement Co. Money
can be deducted on a pre-tax basis to be placed in various mutual funds.
WELLNESS PROGRAMS
RHD is committed to the health of its employees by providing
opportunities for fitness throughout the corporation. Weight Watchers, yoga and
exercise classes, and annual health fairs are a few examples of this
initiative.
