Description The COMPASS Mobile Medication Program is designed to help gain or improve the skills to manage prescribed medications. Our team provides services in the home as well as your community and will work with managing both psychiatric and physical health medication. COMPASS is a voluntary, community delivered services for adults 18 years of age and older living with a serious and persistent mental illness whose illness has presented difficulties progressing towards recovery due to the inconsistent adherence to a prescribed medication regimen. The Mobile Medication Program is a recovery focused service that allows individuals to be educated about their medications and introduced to tools that will assist in developing skills to manage their medications effectively in order to control symptoms and increase community tenure. Members of the team will act as a liaison between the prescriber an the individual in order to help establish better communication and to help integrate the individuals back to a least restrictive environment where they will advocate more effectively for themselves with their prescribing clinician. Participants of this program must be 18 years or older and have a primary diagnosis of a serious and persistent mental illness, (Major Depressive Disorder, Bipolar Disorder, Psychosis NOS, Schizophrenia or Schizoaffective Disorder) and be prescribed psychotropic medications. Participants of this program must be in need of community delivered psychiatric nursing services to prevent the need for more restrictive levels of care and to improve community tenure. Individuals must meet one of the following:   ·         Receiving Case Management services or other ambulatory services and in need of intensive medication management to prevent the need for an inpatient level of care. ·         Current inpatient admission or re-admission due to non-adherence to the prescribed medication regimen. ·         Initiation or revision of a complex medication regimen. ·         Medical diagnosis that requires coordination of Physical and Behavioral health concerns, including medication management. ·         Temporary or permanent absence/withdrawal of a primary support who has been assisting the individual with medication management.
